Welcome to
Alabama Care Plan's
Health Home

At Alabama Care Plan's Health Home, our goal is to help Medicaid recipients with certain chronic health problems stay as healthy as possible. Our specially trained teams look for opportunities to keep you well. Your nurse or care coordinator will guide you on your path to improving your health.
